You are reviewing and acknowledging this Informed Consent because you are seeking Healthcare Services utilizing technologies by the VIM Hyper-Wellness facilitated through the VIM Hyper-Wellness website, iOS mobile app, web mobile app or other technologies collectively the “VIM Hyper-Wellness Platform.” This Informed Consent does not modify or supersede any Terms of Use, Privacy Policy, or Notice of Privacy Practices of VIM Hyper-Wellness or the Providers, rather it supplements these terms and documents.
In-person consultations involve face-to-face interactions where a licensed, certified, or registered healthcare professional provides health and wellness services directly to the patient. These consultations are used for diagnosis, treatment, follow-up, and patient education. This "In-Person Consultation Informed Consent" informs the patient ("patient," "you," or "your") about the treatment methods, risks, and benefits associated with in-person visits to address your health and wellness needs.
In-person consultations offer direct and personalized access to health and wellness services. They allow for immediate, hands-on care and more comprehensive evaluations, ensuring that you receive the most accurate and effective treatment, including follow-up care tailored to your specific needs.
By accepting this In-Person Consultation Informed Consent, you acknowledge that you understand and agree to the following:
1. You have carefully reviewed this In-Person Consultation Informed Consent and understand the risks, limitations, and benefits of in-person consultations.
2. You understand that, in some cases, your provider may be a nurse practitioner or physician assistant rather than a physician.
3. Other individuals, such as support staff or interpreters, may be present during your in-person consultation, and you will be informed of their presence and role.
4. The information you provide during the consultation is considered accurate, true, and complete.
5. You understand that there is no guarantee you will receive a prescription, as this decision will be made at the provider's discretion based on your medical needs.
6. You acknowledge that there may be certain risks associated with diagnostic testing, and some tests may have defects that could affect the quality or accuracy of your care.
7. The services offered during your consultation are specific, and you may need to seek other resources for additional health needs. Providers reserve the right to deny care if it is deemed medically or ethically inappropriate.
8. You understand that while your provider may coordinate pharmacy or diagnostic lab services, you have the right to choose any pharmacy or lab you prefer.
9. You have the opportunity to discuss the consultation, including the benefits, risks, and alternatives, with your provider. You have the right to withdraw your consent to in-person consultations without prejudice to future care.
10. You have access to your medical records in accordance with applicable laws and regulations, and with your consent, your primary care provider or other treating provider may obtain copies of your health information.
11. You agree that VIM Hyper-Wellness is a third-party beneficiary of this In-Person Consultation Patient Consent and has the right to enforce it.
